Ready to join our quest for better?Job DescriptionThe Adolescent and Young Adult (AYA) Oncology Resource Navigator is an exciting, emerging area of oncology that aims to address the unique needs of AYA patients, diagnosed between ages 15-39, to overcome existing gaps in their care and outcomes.
We aim to do this by expanding existing AYA programming across domains of clinical, research, and education/awareness.
The Adolescent and Young Adult Oncology Resource Navigatorserves as an AYA champion across the Northwestern Medicine regional healthcare system.
This role will help bring specialized AYA services and resources to this historically under-represented population and ultimately facilitate more equitable care closer to where our patients live and work in the community.
Responsibilities:
This individual will report directly to the Director of Northwestern's Living Well Cancer Resource Center in the western NMG region while simultaneously receiving direction and regular supervision from the AYA Cancer Program Co-Directors.
This individual will serve as a bridge between the urban academic health center in downtown Chicago (NMH/Lurie Children's Hospital) and the regional suburban facilities.
Serve as a champion for AYA patients and survivors within Northwestern and the community at large to represent and raise awareness for the program including being a key participant in the existing AYA oncology working groups and other program offerings and community engagement initiatives.
Assist the AYA team and relevant constituents in conducting a needs assessment in our regional care sites to support strategic planning and advise future program development.
Facilitate AYA relevant provider education initiatives across regional care sites.
Assist in operationalizing methods for efficiently identifying AYA cancer survivors treated across regions and the optimal methods for outreach to these patients.
Connect with eligible AYA, orienting them to AYA resources and programing available within their region of care, within the NM system and nationally through nonprofit and other AYA targeted organizations.
This may include assisting with AYA-specific financial aid applications.
Serve as a liaison to AYA, their families and the medical teams with emphasis on establishing rapport, normalizing concerns, answering questions and connecting them with developmentally appropriate and compassionate care in accordance with AYA clinical guidelines.
Capable of conducting brief assessments designed to recognize psychosocial risk factors and social determinants of health and refer appropriately to services such as social work, mental health services or financial counseling (i.
e.
transportation, financial aid, insurance barriers; inadequate social support; acute emotional needs; cultural/religious/language barriers); coordinating care as necessary.
Participating in AYA targeted case conferences and AYA team meetings.
Note:
The AYA Oncology Resource Navigator is currently a philanthropically supported full-time position with a 1-year commitment.
This position is jointly managed by the AYA Cancer Program at the Robert H.
Lurie Comprehensive Cancer Center of Northwestern University as well as the Living Well Cancer Resource Center and will be responsible for working collaboratively with multiple satellite clinics.

QualificationsRequired:
BSN, BSW with a LSW, or similarMinimum 3 years-experience.
Combined education/experience as substitute for minimum experience3-5 years minimum experience in healthcarePreferred:
Oncology experience preferred.
